New Patient Registration

To register as a patient, please ask at reception for details.

When you register you will also be asked to complete a medical questionnaire, this is because it can take time for us to receive your medical records. We will offer you a health check with our Health Care Assistant or Practice Nurse.

We ask all new patients for proof of identity when you registering with us, particularly when you register children in your care. Your ID will also ensure you are matched with your details on the NHS central patient registry and that your previous medical notes are passed onto the new practice.

Please bring 2 forms of identification with you a utility bill or such like that has confirmation of your address and photographic identity such as a passport, driving licence etc.

Our Practice Area

The practice is based off Radford Boulevard, Hyson Green and covers Hyson Green, Forest Fields, Stockhill Estate and parts of Basford, Mapperley and Sherwood (see map on right).

Medical treatment is available from the date of registration. Please contact reception for further information.
