Who are Raleigh Healthcare?

Raleigh Healthcare PCN brings together four local GP practices; Churchfields Medical Practice, Greenfields Medical Practice, Lime Tree Surgery, and JRB Healthcare to provide seamless, patient-centered care. As a Primary Care Network, our role is to enhance the services offered by individual practices through collaborative working, ensuring patients benefit from a wider range of healthcare professionals and services close to home. We focus on the needs of our community, working with partner organisations to deliver high-quality, accessible care.

Social Prescriber

A social prescriber is here to support your overall well-being by helping you access community services and resources that can improve your physical, emotional, and social health. They focus on addressing non-medical challenges that may impact your health, such as loneliness, financial difficulties, or lifestyle changes. Social prescribers work with you to understand your needs and connect you with practical solutions in your local community. From accessing social groups to finding help with housing or money matters, they aim to empower you to make positive changes and feel more in control of your health and well-being.

First Contract Practitioner (FCP)

Musculoskeletal (MSK) conditions, such as back pain, arthritis, or joint issues, account for up to a third of a GP’s caseload, particularly among older patients. Research shows that 85% of these cases do not require a GP appointment and can be effectively managed by an experienced MSK clinician. This is where First Contact Practitioners (FCPs) play a vital role.

FCPs are highly trained healthcare professionals, often specialist physiotherapists, who provide expert assessment, diagnosis, and management of MSK conditions. As part of NHS England’s long-term plan, FCPs reduce pressure on GPs by offering direct access to specialist advice. Patients can see FCPs early, allowing for timely interventions and individualised care.

An FCP can assess and diagnose your condition, provide self-management advice, recommend exercises, arrange imaging if necessary, and refer you to rehabilitation services or other pathways such as orthopaedics. These services are designed to provide quick and efficient care, often resolving issues within one appointment.

First Contract Practitioner (FCP)

Mental health practitioners play a vital role in supporting patients whose needs extend beyond local talking therapies but do not require ongoing secondary mental health care. Acting as a bridge between primary care and secondary mental health services, they provide targeted support, early intervention, and specialist advice within the GP practice setting.

Our mental health practitioner works closely with patients to assess their needs, offer personalized guidance, and connect them with the right support or treatment options. Whether it’s managing anxiety, depression, or other mental health challenges, their aim is to help patients navigate the complexities of mental health care with compassion and expertise.

What our Mental Health Practitioners do?

  • Offer tailored mental health support for adults and young people.
  • Provide early intervention and strategies for managing mental health challenges.
  • Collaborate with primary and secondary care services to ensure holistic support.
  • Facilitate referrals to appropriate services, such as talking therapies or community mental health teams.
